How to Apply for a Grant

Grant Request Submissions will need to be submitted online.  If you are unsure of your eligibility to apply for a grant from VNA Foundation, please first contact us at info@npvnafoundation.org to discuss.

  1. Your organization should be a tax-exempt 501 (c) 3 nonprofit and should have filed at least one form 990 before applying for funding from the VNA Foundation.
    To apply online, access the “Online Grant Application” link at the top of the foundation’s homepage, and choose “Requests”, then “Apply”. LOI/Applications must be completed and submitted online as instructed, complete and on time in order to be considered. In the Spring, an LOI (Letter of Intent) is required prior to the completion of an entire application. This is also an online process. New applicants: Access the online grant interface and register with your email address and a password before proceeding. Once into the system, you will have a desktop which will allow you to “Apply” for an available grant opportunity. (In between grant cycles, there will be no way to apply for grants online.). Applicants Already in the System: Access the online site and select “APPLY” from the list of options in the left-hand column to start a NEW application.
  2. Proposal Application will include the following detailed information:
    • Summary of the Project
    • Demonstration that the foundation funding will address a specific need or issue within the general community, and does not duplicate existing efforts
    • Goals for the project and projected timeline
    • Expected Outcomes (Measurements of Success)
    • Mission Statement and History of the Organization
    • Number of Clients Served in Foundation Coverage Area (Boundaries of 3 school districts: North Penn, Souderton Area, and Wissahickon, plus Worcester Twp.)
    • Collaborative Partnerships Involved, especially those involved in the project under the grant request, and within this North Penn region
    • Other funds applied to project or grant request are encouraged and should be documented in the narrative
  3. Attachments should also include:
    • List of Board officers and directors
    • Budget Form  – Showing organization budget and specific project budget figures
    • Most recent independent financial audit and management letter
    • Latest Strategic/Business plan (if available)

Note: All grant recipients will be asked to submit both an interim and a final Report during the grant period.
